adding device to threshold manager using ciscowork for window 6.0

I am have a very difficult time trying to add a device to the threshold manager. I have a cat4006 and cww6. I have created a template in threshold manager call 'cpmCPUTotal5sec' to monitor cpu utilization on a c4006 switch. Everytime I tried to add this template. It did not show up in the 'device theshold' window. I have created other template such as 'sysTraffic' and was not able to add it. I get no error. I really appreciate your help.

If you're using pre-configured templates, you need to click on Add to device to add that template to the device.
