You don't say whether this is a 56/64K CSU/DSU interface or a T1 CSU/DSU. You did say it is an RJ-45 cable which indicates 56/64K, and here is a link with the pin-outs (scroll down a little):

http://www.cisco.com/univercd/cc/td/doc/product/access/acs_mod/cis3600/hw_inst/cabling/marcabl.htm#xtocid20

Just connect pin 1 at one end to pin 7 at the other, and pin 2 at the first end to pin 8 at the other.

Mark . What if it's an T1 dsu/csu ? The link seem to imply a straight throught CAT 5 cable will work.

then how about a router with a old style serial interface (cisco DB60) to a router with a T1 DSU/CSU ?

A UTP/Cat5 T1 crossover is 1&2 crossed to 4&5.

A DDS/56k/64k UTP/CAT5 crossover is 1&2, 7&8 (also used for ATM 155).

There is no way to do a three-way (with a cable)... only point-to-point.

If one of the routers has two serial ports, you're good to go.

Good Luck
