Three weeks ago I have upgraded one 6506 software
CatOS 6.4(3) + IOS 12.1(13)E6. No problem!
To use the GBIC-TX, jesterday I upgraded the same switch at the CATOS to 7.4(3).
Then appears the problem that the module Multilayer not come online!
2003 Jun 05 19:53:39 %SYS-3-MOD_FAIL:Module 15 failed to come online!
The status of module is other!
No comunication beetween Supervisor and Multilayer (but I can log via session 15).
The workaround to bring the Multilayer online is to reset the module 15
from the catalyst!
I have upgraded another 6506, same releases same problem.
The power-off power-on don't solve the problem

Andrea,
Did you try putting the console cable on the supervisor and type in switch console to see what the MSFC is actually doing ?
Did you try change the boot statement prior to the reboot for the MSFC ?
I would definitely like to see what is the state of the MSFC. Most properly keep trying to boot your previous boot image.

I was connected to the console. The Supervisor showed
that Module 15 failed to come online BUT I COULD entry in the msfc trough the SESSION 15 AND THE MSFC was UP, BUT ALL THE VLAN DOWN!

Most probable reason is that the MSFC is in rommon. The following page should help you in recovering it
