Catalyst 4006 Module Fault

After a recent power outage, a 4006 switch showed all connected ports "faulty" on blade 5; this blade is MT-RJ. The rest of the switch modules were working okay, so I performed a reset on blade 5, which restored the port connections.

Has anyone else encoutered this? Should I be concerned?

You are running 6.1(1) which was the first release on and is no more on CCO. I suggest that you upgrade to 6.4(1), set the diaglevel to complete and reset

Can you paste the output of sh version. Also upgrade to latest 5.5(x) which is 5.5(18) or 6.4(1) as these versions have more intensive diagnostics. Also set the test level to complete using "set test diaglevel complete" command and reset the switch to check for any flaky hardware
