The Cisco2621 has a serial connection to a Server via X25.Each time when the Server on the serial port reboot. the router will enter rommon> and need to type b to boot it.
Is it caused by a faulty serial cable or something wrong in IOS(12.2(2)T) or memory problem? Appreciate any comments.

I suggest you open a ticket with TAC. Sounds pretty buggy.

Good news that the problem has been solved. The router is working after change the power outlet for the server and isolate it from the one for router. Seems that when the server is down, it draw the power level of the router power supply as well. A faulty power outlet.
