Catalyst 6509, redundant SUP2 engines, Native IOS 12.1(13)E, three WS-X6816-GBIC cards, plus other modules. As per documentation, GBIC integrity test is performed only on copper GBICs. With our customer's switch, 1 of the 3 GBIC modules had Pass (.) results instead of Unknown (U) for some of its ports (using 'show diagn module all' command). Is this a problem or not? I really don't think it is a problem because the switch is performing well without any problems but I would appreciate any advice.

I also think this is not a problem. Pass and Unknown are ok. The only ones you will have to be worried would be : Loopback failure, Stub failure, Ilc failure, Port failure, SEEPROM failure and GBIC integrity check failure
