I have seen, in many design, a single server connected to 2 switches to avoid that a fail in just one switch stops the server. But, what in the server does make the interface fail over?

Many companies make NICs capable of failover. Compaq and 3COM do. They also produce nics that team or channel together. I don't believe that Cisco does though. I would check out compaq.com for more information.

Usually loss of Link . The driver code for the NIC sees the physical failure and moves the traffic to the standby link.
