Is the Local management interface the router on my end, or the isp's dce equipment ?

Both. It's a bi-directional protocol and needs to be present at both ends.

What actually sets up the call, does the LMI switch at the isp's end tell my router which dlci's to use and my router then sets these up ?, can you give me a simple quick explantion how it is set up ?
