Mode conditioning cable ( MCC )

When using an LX/LH GBIC with 50 micron MMF, do I have to install a mode-conditioning patch cord?

If that is the case, what is the ordering number of this patchcord? Only found CAB-GELX-625 for 62.5 micron MMF.

No, it's only 'suggested' for 62.5 micron. I bought a few since I needed to run over 62.5 for 300m. I couldn't get a reliable connection using them, and the connection works greate without them.
