Need HELP 2950 Erroring out.

My switch was working fine until around 7-8 PM MST on Sept 27.

Ports 1-16 are flashing green and amber a very fast rate.

Checked console and here is what I'am seeing:

error intializing transceiver ports

cmic_schan_op ERROR: timeout (OP=0xD polls=100000

This flashes by over and over. As with this:

Dunno of hardware went bad or if this is software. Any help is greatly appreciated.

It is a hardware failure. Open a TAC case and get it replaced.
