The pa-8t card only allows a total of 128 channels configured and only 5 full T1 PRIs can be configured. I remember a Cisco doc describing this limitation but I can't find it. Anyone know where it is? Does Cisco intend to fix this to allow all 8 ports to be fully configured?

Do you really mean ISDN or do you mean ds0 channels in a fractional T1 ?AFAIK this PA cannot be used for ISDN but only for serial interfaces, i.e. fractional or full T1/E1.
According to the specs:
Eight full-duplex ports at T1 (1.55 Mbps) or E1 (2.048 Mbps)
Four full-duplex ports at up to 4 Mbps each
Two full-duplex ports at up to E2 speeds (8 Mbps)

mentions that it can only handle 128 channels; the 5 PRI limit is not explicitly stated but since 6 x 24 > 128 this is implied.
The PA-MC-8TE1+ can do the full 192 channels used by 8 PRIs:
