Hey,
I just unwrapped a new CAt4006 with a sup 4013 and a couple WS - X4148 RJ modules. When I insert the module in slot 4 , I get the following messages:
%SYS-5-MOD_INSERT:Module 4 has been inserted ( I did it !! )
%SYS-4-P2_WARN: 1/Module 4 is taking an unusual amount of time to come online
%SYS-4-P2_WARN: 1/Astro(4/2) - timeout occurred
%SYS-3-MOD_MINORFAIL:Minor problem in module 4
And also I get the following result with a show test:

I suppose the chassis is faulty ! But the customer is waiting, can anyone provide an explanation or workaround ?

Try the card in a different slot other than 4 and see what happens. It could be a bad Supervisor. If possible paste the output of sh ver

I tried different cards in different slots, and only in slot 4 every card didn´t work properly.

Ok, and now the big surprise: My colleague reloaded the Switch several times, and now it works . But I´m still concerned about it, this is not a good solution.

Upgrade CAT OS to 5.5(15) since it has more error reporting functions and does a better job in testing hardware for any failures. Also make sure all the cards/Sups are fully seated and thumb screws tightened
