You could be hitting the following bug CSCdx87646

A reset will fix the issue, but the config will be lost upon reset. Back up your config to a tftp server and/or to the switch bootflash using "copy config" commands. 6.4(1) and later in 6.x will have a fix for this issue

You are hitting the bug i referenced. In the sh ver, I see "Global checksum failed. ". I suggest that you back up the configs and upgrade to 6.4(2)
