Most likely what is ccausing the bad voice quality is insufficient QoS configurations to guarantee a strict priority for voice packets, and also fragmentation of large data packets if the circuit speed is less than 768kbps. You can read a QoS design guide for IP telephony (also applies to non IP phone installations) to get an idea of some things you will need to look at doing.
