WIC card failure

I'm working with a 1602 with an internal 56k CSU. Tested the device in house with no problems. I sent this device to an installer in a remote location, but was unsuccesful getting the circuit to come up. This was to replace the current 1600.

When I got the device back, the interface would come up for approx. 1 min., then go up/up (looped).

My question is, has anyone ran across a situation where the wrong cable plugged into an internal CSU damaged the CSU?
