What IOS version is the switch running? The message:
Feb 10 22:32:41.262 UTC: %SUPERVISOR-3-IIC_INIT: IIC Reinitialized
What kind of media converter is on interface gig 1/0/1? I am thinking it's related to that interface flapping. Is the error peristent? If it is, does the error stop when you shut down the interface gig 1/0/1?

Media type is 1000BaseSX SFP
IOS version is 12.2(25)SEB4
It disappear after a reload. But I really want to find the root cause to prevent it happens again. It bring down the the interface everytime it happens.
do you know what does "IIC Reinitialized" means?

Check out bug:CSCee88632 too. I can only say that interface controller register access failed because of IIC bus lockup - maybe due excessive flap of the interface. switch s/w cannot do anything under such circumstances but log an error.

bosalaza,
You could be right. Nobody touch the switch during this problem occurred. I may need to research more on this issue. Thanks
