Hi!

Do you have any other WS-X4013 supervisor Engines? Try to insert a new one into the Chassis and check if the failure still exists. If so, you can make sure there's hardware problem with the original one.

Or insert the WS-X4013 to a new chassis to watch the POST, if the problem still exits, you can deduce that the slot for WS-X4013 of the chassis has some problems.

I use show test command, does that mean that 4013 need two power supplies?

Definitely, you need at least 2 power supplies to get the switch run normally.

The following info is from Output Interpreter

SHOW TEST NOTIFICATIONS (if any)

WARNING: One or more ports have failed the loopback test.

TRY THIS:

1. Try reseating the Supervisor module and resetting the switch

2. Try the supervisor in another slot and/or chassis

3. Isse a 'show flash' and make sure that the MCP and LCP codes exist and are

up to the latest revision, if not, re-download the NMP code.

4. Check the temperature, power supplies and fans using the 'show system' command.

5. If none of the above solves the issue, contact Cisco TAC for additional

assistance and/or replace the supervisor.

It appears that memory has failed and the Sup will need to be replaced. Try power cycling the switch once to see if it matched CSCdy46288
