Often times complex troubleshooting issues are best addressed in an interactive session with one of our trained technical assistance engineers. While other forum users may be able to help, its often difficult to do so for this type of issue.
Looks like your file system is corrupt, it happens occassionally.
You may need a replacement, but if you call the TAC they'll tell you to start a console session (not Telnet or WWW, but plug straight into the console), login and then at the menu prompt type 911. This is a hidden option that will stop all concentrator functions and put you in a special mode, so be sure and do it after hours. After logging in you can "fsck -a" the hard disk and it might fix it up.
Be very careful in here, don't go playing around with options that you're not sure of. When you're done, do "reset" to bring the concentrator back on line.
