Hello Reza,
Were there any recent changes to the Pix? Was this Pix ever operational?
Initial thinking is perhaps you have corrupt OS. If so, do you want to re-image the Pix via monitor mode?
Here are some instructions
Enter Monitor Mode on a PIX 501, 506, 515, 525 or 535

Hi,
The OS of the PIX might be corrupted. I suggest reimage the PIX.

Thanks for your prompt response. It was working well and kept as backup. Nothing changed.

I had the same problem, but atfer several reloads and cutting off power, it started to enter Monitor mode. First it enerts Monitor directly (without giving the option to boot from flash), hence I thought I need to reinstall the OS, so I did that from Monitor mode using the tftp method. The option to boot from flash started to appear and the OS was booted, not from the image downloaded from the tftp server but rather from the one saved originally on the flash; I discovered that after I removed the image on flash to make space for the new image. A strange thing has happened, once I removed the image on flash the PIX reloaded giving me no chance to save the new image, and thus the PIX got into a boot loop and every time I try to install the OS from Monitor mode the image is downloaded but the OS doesn't boot and the loop continues.
My PIX was working fine as a Primary in a failover configuration. It became non-operational after a power failure has occured, so my best guess is that a hardware failure is the reason.
The problem is of high urgency for us, so any help will be highly appreciated.
