I had this issue when I first started setting everything up. First, what model Controller do you have and what software version are you running? My 4402 was running software version 4.xxxxx out of the box but Cisco TAC told me that the 1142N AP needs software version 18.104.22.168 or higher to register correctly since it uses CAPWAP. Second issue, is your Controller and AP on the same subnet? If not, try putting it on the same subnet after updating the software version to make sure it registers correctly. If you plan on putting the AP on a different subnet you will need to configure Option 43 in DHCP.
