I have a Cisco ANT2506, which is clearly marked as a Cushcraft product. However, Cushcraft doesn't have anything in their product line that matches the specs of the ANT4121. Does anyone know who actually manufactures that antenna for Cisco?

Cisco only manufactures the product Cisco's ANT4121 antenna.

First, the picture in that PC Connection photo is of Cisco's 2506 antenna, not the 4121.
Second, it's highly unlikely Cisco *themselves* makes the 4121 antenna. They don't make the 2506 - they OEM it from Cushcraft. This was my original question... who is the ACTUAL manufacturer of the Cisco 4121 antenna?

Cushcraft probably make it for them. It is possible that that, by contractural agreement, Cushcraft does not market their own version (Cisco only).
Cushcraft is one of the largest antenna manufacturers in the world.

It could be made by Telex or Xirtex.
