Remote Access via Analog Connections to destination having the same ip .

I have the following customer requirement:

1- Many clients in LAN should be able to connect to Systems on different remote destination.

2- The remote destination are pc's connected to modems. These pc's have static ip on the interface connected to the modem. The static ip is the same for all destinations( always 192.168.1.1 on the remote pc ) it doesn't matter in which destination this pc is. The PC's are only different in the destination Tel.Nr.

3- A connection from another client to a different destination works in the same manner as in point 2.

The requirement is to build a central system that do this job.

Is there any known solution for this requirement?
