Problem with SF300-24 switch

This switch has been functioning well for about 3 years.  It has recently been taken out of service as it has stopped working properly.  I tried getting into the management console via http.  I logged in, then it goes to a page with 3 empty boxes at the top with no description of what it is requesting with a submit box with nothing inside it as well.  there is no left menu system.  Nothing else really to click on, other than the cisco link at the top to go to cisco.com webpage. - screen shot attached

Can i assume that it is beyond repair?  or can i assume that the firmware is toast, and i need to somehow flash it?

any help would be greatly appreciated.

Problem with SF300-24 switch

Hi Paul, first I'd recommend to try a competely different computer, wire and port to try to manage the switch, if the same result, reset the switch by holding the rest button in for about 35 seconds. That should clear it up.
