For SQL authentication the username is normally "sa" unless disabled by an admin. Integrated would be if you have an AD account for the SQL server.
The password is set by the one setting up the SQL server (if setup by the TMS installer then the one installing TMS should know the password).
If you log on to the TMS server (or where the SQL server is) as administrator, open the SQL server and log on as administrator via SQL Management tool, no password is needed in this case as it uses the current session. From here you can expand the "Logins" and find the "sa" user, go to properties on this user and change the password, then use this user and password for the installer.
