Emergency Responder

I have 2 Emergency Responders (both 1.2.(3a)sr2a)that have the same database info.Customer started to receive alerts that an Ext.generated a E911 call.This is confirmed in an "Emergency Call Alert" entry but not in "Call history" entry.CDR of Call Manager does not show Ext.making call,nor does enduser confirm 911 call was made.

Any ideas??
