Not hearing Unity

Intermittently when responding to a voicemail message the user presses # after their introduction but do not hear anything. According to the trace files Unity is asking them what they would like to do next but the user is unaware of this. If they stay on long enough they may finally hear her. The user is not sure if the reply was send or not. The replies are being sent. We are CCM 34.0 and Unity 4.0(3). I have put a sniffer on but it is not an easy task to capture. Anyone have an idea, what is going on?

Sorry, I should have mentioned this only happens on external calls.
