when I VPN to my office from my home and then try to use Personal communicator to dial Internal extensions then I have no problem, I can hear internal user and they can hear me, but when I try to dial external number then I can hear them, but they can't hear me. Any help will be appreciated.

Do packet capture on both ends. See if RTP is flowing in both directions.
If one end sent out RTP, but the other end didn't receive it, it's a network problem.

I can capture the packet on IP phone site, but how to capture the end which is PSTN mobile phone! My network looks like!

You don't have to capture from PSTN side.
Just look at the packet capture from the IP phone side. See if it receives any RTP packets. If it didn't receive any RTP packets, then it's not its fault. :)
If you really want to trace down the problem, you need to do packet capture on the voice gateway.
