Recreating the SQL Maintenance Plan in Unity

Client recently upgraded to 4.0(4)sr1 and since the upgrade the DB Maintenance Plan is failing every night so the optimization is not running. Anyone know how to "recreate" the maintenance plan or have a good doc on exactly what its supposed to do.

Do you mean the nightly and weekly backup jobs? If yes, please post the app logs.
