Transferred to FailSafe conversation

When users access their voice mail, and hit 2 to Send a message, the system disconnects with a message "This system is temporarily unable to complete your call.." and disconnects.

The App log has several items from

Event ID - 10013

Description:

Transferred to FailSafe conversation while running conversation AddrSearchConv on Port 42. See the following file for the contents of the Named Properties : e:\CommServer\Logs\\NPDump_20070814_091536.txt

This is closely followed by PhraseServer Errors;

Cisco Unity's Phrase Server Manager experienced a serious error.

Hi - hope you don't mind me chiming in :-)

Customer is running Unity 4.0(4)SR1 with Installed Engineering Specials: CiscoUnity4.0(4)_ES23, CiscoUnity4.0(4)_ES35, CiscoUnity4.0(4)_ES46 and Exchange in failover configuration. From the attached log file.

Resolved issue. Problem was related to wav files used by the directory handler.
