There is a problem with my Unity. Some of the voice messages are just 'hung up' tone, and up to more than 5 mins (or the max voice msg length). It seems that the Unity is unable to detect the call has 'hung up' and keep on recording the v msg.

can you state just what the 'hungUp' tone sounds like?
is it a busy tone? fast busy tone? dial-tone? dead-air?

I have a similar problem. When receive a call from outside our network, get dead air when the Unity greeting is supposed to play. We have Unity 4.0(4).

Hi -
Have you confirmed the call is reaching Unity? If using CallManager integration, you can run Call Viewer or if non-CallManager, run Integration Manager from your Unity server to see calls coming in from your PBX. In your example, when a call comes in offnet, does the problem occur when the caller rings a subscriber phone and rolls to voicemail coverage? Or is the caller calling the remote Unity access number and not getting the Opening greeting?

When 1 party of a call has hung up/terminiated the phone, the tone that the other party hears after the hung up. It is strange that some of the v msg are "dooooo-dooooo-dooooo-dooooo" and so on (for 300 seconds; default voice message length).
whereas for other v msg, it records up till the calling party hangs/terminates the call.
