Figured it out with the help of colleagues and TAC....
There seems to a feature in Cisco Unity connection 7.X where the UI administrator password expires by default and unfortunately needs to be reset through the CLI. You can prevent this by manually marking the web application password for the ui administrator to never expire.
If you haven’t done this and the password expires use the following to reset the password.

The below trick might come handy when you have to add a new node to a cluster but you don't have or is unsure of the security password for the publisher. This procedure has been around for ages.
1) Login into the CLI of the Publisher.
