We have a 4.0.4 in a MS Exchange store environment and received the following error: Description:
Cisco Unity's multi-media component has enountered a serious error.
EXPLANATION:
A serious failure has occurred on port 32 while accessing the RIFF Header of an audio stream. Depending upon the severity of the failure, parties on this call could experience unexpected behavior or be disconnected. In some cases, further calls on this port will not be handled correctly.
TECHNICAL DETAILS:
Thread 0x0000138C had a Failure on Port 32 in AvWav
DESCRIPTION:
File: e:\views\cs_ue4.0.3.159\un_Miu\UnityAvWav\WAV.C(1084)
Method: WavOpenEx
Failure: call to mmioDescend failed with error(109).
-Does anyone know what this is or how to respond to it? Testing of the port in question has been fine.

I've seen similar errors (don't have the exact ones anymore) when we had a utility on our mail servers that was zipping .wav attachments. Our email server administrators didn't exclude .wav files, so everyone's Unity messages got zipped and they were unreadable by the Unity server. Fortunately they were able to unzip them and things worked fine again. You might want to look into that.

I've also seen various anti-virus packages cause problem opening and streaming WAV file attachments that give similiar errors.
