Unity VM from external callers not working for 1 user

Call manager ver 7.1.3

Unity ver 7.02

Domino Server version 8.5 HF424

We have one user who's voicemail is not working correctly

User can receive voicemails from interal call manager users. However if an external caller (PRI call)
leaves a messages (they get the users greeting and can leave a message) but there is no record of the user leaving the
message when the voicemail box is checked.

We tested this using a mobile phone and left multiple messages for the user but no alert
is sent or MWI and if the user checks their voicemail there are no new messages.

We've recreated the user multiple times cheked unity server permissions on mailbox, marked for reimport into unity but it still doesn't work

Any help would be greatly appreciated.

We run Unity 5.0 with exchange. i have had this happen a couple times when we restart our domain contoller. very strange in that its only external, it impacts us a bit more in that no external calls are able to leave message, even though they are prompted to. i usually fail over to standby unity and reboot primary, fail back and all messages thet were left come in and all returns to normal.
