Unity voicemail PIN reverted back to default after changed

Unity 4.2.1

New subscriber logged in in to voicemail with default PIN and got prompted to change the PIN. The new PIN however reverted back to the default PIN next time the subscriber logged in.

Hi -

On the subscriber's Profile page in Unity SA, is this box still checked - "Set subscriber for self-enrollment at next login"? The user may have hung up before Unity completed the enrollment process.
