Since I've tried everything else I can think of and find, it looks like I may have to save/delete the user's Exchange account and start over.
When I worked with TAC on this, they did enlighten me about the Bulk Subscriber Logout utility. Running that first before you migrate users to a new mailstore should alleviate issues like this, so hopefully that will help someone.
