We have a PIX that sits between the Data VLAN (PCs) and the Voice Vlan (Phone and CallManager). Does anyone know what ports (TCP & UDP) we need to open on the PIX to allow the Attendant Console app to run from the Data VLAN (PCs)?

With restrictions imposed on the PIX and logging enabled, I would suggest you to initiate the session from the Attendant console, you will then see this traffic being blocked by the PIX in its log with the Port # information. You can then open up those specific ports for the particular IP address.
