Sounds a bit odd. You'll need to look at the CUDLE tool in Unity Tools Depot or directly in SQL but before you move forward, you will need to take a look at whether or not there is an alternate extension set up for a subscriber that equals the extension of the phone. In addition, you'll need to check for alternate MWI extensions as well. If you have discrepancy between Unity and CCM on the MWI, I would suspect that you shouldn't find this to be the case but nonetheless, you would want to rule that out as well.
