Name is managed by the Telco. The call manager system can only manage the outbound number presented to the PSTN. the Telco takes that number and then references their database to find the name to display on that call
It also depends on where you are. In some parts of US and Canada, telco will happily deliver whatever name and number is given from PBX. I've always thought that gives a chance for fraud and pranks, but hey, that's the way it is.
