Tandberg disconnects on Cat6500 6148AF blade

We have a Tandberg MXP unit that disconnects intermittenly when connected into a 6148AF blade. When we use a 6348 blade or 3750 switch we do not have issues. We've disabled port security, unicast packet flooding, inline power but it still disconnects. The port remains enabled but does not detect the connection. To get it to work again we have to disable and reenable the port. We have another Tandberg on a 6148V blade and that one works fine.

Has anybody seen this? Any recommendations on what we can try?

Does the system thrw any kind of messages during disconnecting? You can get more information on cat 6500 troubleshooting from the following link,

Unfortunately, we don't see anything on the switch. It just seems to think that a device disconnected. However, our logging is set to defaults so we'll be setting that to show everything.
