IOS "plus" doesn't exist anymore for new versions. The T train is instead to deliver new features, but in you case you should be fine with 12.4(16) non-T.

I had upgrade the new IOS ver 12.4(16) but it still problem.

analog phone --->Cisco IAD 2430 --->MPLS Network---->Cisco As5400 ---->PSTN.
Problem occur in analog phone to connect Cisco IAD 2430
Does it happen all the time or just sometime ?
How much traffic are you sending over the mpls network ?
Can you keep a continuous ping and check that there is no loss of connectivity when the call drops ?
Do you use CCM with MGCP? Which exact version of CCM? Can you send a diagram of the whole voice network ?
Also you said you upgraded do 12.4(16), but in the attachment we read:
please take the show version and show run again.
I did not use CCM with MGCP,I use BroadWork.
When I upgraded to Ver 12.4(16),problem with all my phone.when conversation,I speak but in here did not hear any thing,then I restored old version 12.3

With MGCP, in all honesty I'm unable to help you further. It is possible that there are settings needed to make the IAD to work with broadwork, but I wouldn't know which ones.
The fact that you had no voice when upgrading to 12.4(16) may be due to the need of such specialized settings.
At this point, you may ask the TAC to help you (need a support contract) or ask again into the "ip telephony forum" for a problem with MGCP broadwork. In both cases you will be required to provide MGCP traces.
