VoIP session filling up ports on PRI and VWIC-1MFT

I'm having a problem with getting occassional busy signals on my IP  PBX. I've narrowed the problem down and occassionally I get duplicate  ports being consumed on a single call. With only 1 RTP session I can get  up to 9 channels consumed on my PRI.  I have verified the cable used is  correct per the adtran manual and cisco manual for the VWIC-1MFT  (crossover PRI pins 12 45 and tests ok on the cable certifier). Problem  appears to be completely random and I have not been able to duplicate  it. I have verifiedthe PBX has only 1 SIP/RTP session between the itself  and the VoIP phone. It appears to be occurring on the Cisco 1751-V. For  Privacy I have edited he calling party and destination numbers to  correspond with the call. I'm wondering if someone a bit more  experienced can help point me in the right direction of what to check  next because Im at a loss.

VoIP session filling up ports on PRI and VWIC-1MFT

I have verified everything I know and still nothing turning up, no errors, no alarms, nothing. So I'm now leaning towards a bad piece of equipment since I have similarly setup gateways in use and they are not suffering from the issue. I'll start replacing the cards, DSPs, and ultimately the chassis and see what happens.
