If possible can you do a full reload of the system and then try again?
Also can you give us a snapshot of how many ephones are created? Would be good to make sure that you do not have an excess amount of ephones already created that are not being used.
Also was the users licenses managed by CCA or did you do it manually? Or maybe a silly question did you modify the Max ephone # count to 40, or did this happen during a software upgrade??
Any further info you can give would be appreciated, even a running configuration with all the sensitive info like user/pass removed would be good as well... Please attach it as a txt file and do not post it directly onto the forum.
