SPA504G Randomly reboots disconnecting connected computer

I've had a problem for a while now with a SPA504G, actually this is the second one I have had tried. 

The phone will reboot randomly throughout the day every couple of hours. Got the computer connected to it. I got about 20-25 phones on our network, we got 30 licenses so it is not a licensing issue. 
It is only with the extension #340, I have had a phone plugged in there with extension 116 and that worked fine without reboots. Keep in mind I've used 2 brand new phones with the 340 extension.

The first time removed the phone from CCA completely, scheduled a reboot after hours, then connected the phone and assigned an extension. That lasted for about 4-5 days then it started happening again. I've tested the cables that goes from the phone to the switch as well as the ports on the switches. All checks out. 
To be honest I Have no clue what the next step for me to take would be;

In the reboot history it shows "VLAN Changed :(TIME)" Can happen as much as 3-4 times in a row within 20 seconds and then work for an hour. 

Anyone know what my next step in the troubleshooting would be?
