VIC3-2E/M Card stuck

I have experience E&M(VIC3-2E/M) card stuck, user cannot make E&M call and required to restart their CISCO Router.

The problem was not from CISCO Router as router works normally, user still  can have internet connect.  PABX also work normally. Physical cable connection was checked, also okay. 

Can you advise what problem caused that?  And how to avoid the problem?

Which version of IOS and Chassi did you useCould please send the <show diag> or <show inventory>.
