I have a 2621 which does not recognize the NM-16ESW module, below is output from bootup and sh ver:

Tried the module in a 2610 with the same results. I know the module works!
Do I need to update the boot rom??
Excerpt from Cisco website:
The Cisco EtherSwitch NM is supported on Cisco IOS Software Releases 12.2(2)XT and greater versions only for the Cisco IOS 2600/3600 Series Software
Anyone have any ideas?

12.2.31 is GD release and does support the module. Can you verify that the NM is seated properly on the slot ? How do you know for sure, that the card works. It could be a bad card.
Can you try a memory-size iomem 10 from global config mode and reboot and see if that makes any change.

NM is indeed seated correctly; I've tested the card in a 2611XM that's running 12.4
Adding memory-size iomem 10 has made no difference.
As I have only 16Mb flash I've been unable to use 12.3 however I've justed tested booting c2600-is-mz.123-15b via TFTP which has recognized the NM card.
Seems there's an issue with 12.2.31
Will boot 12.3 via TFTP until I get get the necessary boot rom upgrade & 32Mb Flash

Unless something is wrong with the slot on your 2621, do you have another NM that you can put on the 2621 to see if the slot is ok?
Also can you try loading 12.2.31 on 2611XM (that worked) and see if the NM-16ESW is supported on that ? (Not sure if there is a 12.2.31 GD available for the 2611XM, that you can download).

According to the Software Advisor http://tools.cisco.com/Support/Fusion/FusionHome.do, it should work with your software. In fact, the SA actually mentions c2600-is-mz.122-31.bin as the current supporting version. The minimum in GD IP-Plus is 12.2(26b).
So it looks like there is something wrong with the module.

You should try 12.3 mainline code with IP plus feature set. IOS Roadmap suggests 12.2(2)XT is evolving from 12.2t train. All the 12.2T train IOS are migrating to 12.3 mainline. You can try upgrading to the latest in 12.3 mainline. Make sure you choose IP Plus. Also make sure of memory requirments. IF the module still does not show up, replace hardware. It could be bad slot. Try a different chassis.
