4006 module issues

After 10 minutes the WS-x 4148 -rj45v goes offline.

what can be the reason?

Looks like you have a faulty Supervior module, that would be why the other module goes offline, it need the supervisor to run.

Mike's right.  Your Sup is ... how do I say this, faulty.
