8 Port Bri is not working

We had procured 8 port bri but when i installed on 2611 router it is disabling the slot.See below show diag output & sh ver.Please suggest if any issue with IOS or router.

Slot 1:

BRI (S/T) Port adapter, 8 ports

Port adapter is disabled

Port adapter insertion time unknown

EEPROM contents at hardware discovery:

As far as I can find, this combination is indeed supported. RAM is also adequate.

Could you please log all output during boot and post it here?

This might give an idea of what is the problem here.
