Advice on network design.

I have a design question which I'd like some advice on. I have a router sitting at a hospital. This router has to connect to two different office suites via fiber. Both suites connect in the same closet and the fiber to the router is a few blocks away. Do I purchase a switch with layer 3 capabilities or should I do a "router-on-a-stick" and vlan them? I'm not sure of the bandwidth needed so I am swaying toward the L3 switch to subnet the two suites. Any advice and suggested parts would be helpful! Thanks.

Is this "dark fiber" (i.e., not to a carrier, just glass between the hospital and the offices)?

What routers are in-place already?

What kind of traffic are you expecting? (VoIP, X-Ray, productivity ... all of the above)?

What kind of security is required? Have you compared the overall setup with the HIPAA requirements?

This would be helpful information ....
