Begin with show controllers and check clock config, what exact hardware is that?

You can place the circuit in loop then the burden is on telco to prove the circuit OK.

Show controllers T1 or E1 depending on what you have. Also under controller you would set the loopback local.

One possibility is that some data structure has corrupted and causing the instability. An in-depth look would be needed to see what exactly is going wrong.

Or you can upgrade to the latest of 12.4(9)T and reboot at the next occasion. Chances are, it will go away.
