Destination-based NAT

I'm trying to configure NAT to translate destination addresses but no luck so far.

Essentially, traffic originates from a server going to a destination address of x.x.x.x (outside local) and this destination should be translated to y.y.y.y (outside global)

I tried using the following config without success:

ip nat pool TEST y.y.y.y y.y.y.y prefix-length 32

ip nat inside destination list 100 pool TEST

access-list 100 permit ip any host x.x.x.x

I've read in a few places that you need to use route-maps to achieve destination-based NAT but if anyone has any ideas they would be appreciated.
